Fayette County Department of Job and Family Services Family and Children first (FCFC) Service Coordination Coordinator in Washington Court House, Ohio

Service Coordinator provides Service Coordination on behalf of the Family and Children First Council.

  • Service Coordinator organizes themselves and others to hear and respond to the family's experiences and establishes the family as equal partners in the overall plan process. Structures and facilitates team meetings throughout the process in such a way that honors and prioritizes the listening, responding , and planning to the lived experiences of the family.
    • Contacts referral sources
    • Coordinates a team of providers in order to develop a family centered service plan.
    • Engages with youth and family to identify natural and professional supports.
    • Facilitates team meetings. 
    • Enters required data into case management system.
  • Service Coordinator assesses families as defined in the FCFC's Service mechanism to assist in the determination of the child and family's strengths and needs as part of the coordination plan creation. 
    • Interacts with families focusing on the strengths they have to overcome the needs.
    • Follows timelines for assessments and case management as required per agency guidelines.
    • Explores the services, supports and interventions that have previously helped and during team meetings facilitates discussions on these positives can be utilized to create a family focused plan of care.
  • The Service Coordinator monitors the Plan of Care and the Service Delivery.
    • Works with the family and team to collect data to help determine if services, supports and interventions are making a positive difference.
    • Explores with the family/team inclusion of potential community based and natural supports that can both help meet the family's goals and build a sense of community around the family.
    • Regularly meets with the family/team members to discuss if plan is effective and identifying any potential barriers that may exist.
    • Regularly updates cases and assessments to monitor family progress and adjusts plan accordingly.
    • Shares results of collected data and progress during team meetings to evaluate strategy of plan in meeting the family's goal.
  • The Service Coordinator focuses on establishing the help necessary for the family's continued success.
    • Coordinates the establishment of a transition plan for after disenrollment from service coordination.
    • Captures and summarizes positive changes in the family's  ability to manage challenges and access supports.
    • Works to customize services and resources necessary for a smooth transition.
    • Ensures that family knows how to re-access services in the future if needed.
  • Provides supportive services to the Children Services Division
  • Uses SACWIS, OASCIS and agency computer system to record  and review case information. Scans and documents information into TRAVERSE/EDMS.
  • Completes other tasks as assigned
  • Maintains confidentiality
  • Works closely with families in times of difficulty often with a need for a variety of services
  • Conducts regular meetings with members of local agencies, service providers, and support programs
  • Presents materials and information in a clear and concise manner.
